A diagnostic will typically only be effective if there's a MIL or check engine/service engine soon light on. It operates through a series of sensors throughout the powertrain, so yes there can be potentially serious problem the computer won't know about if it's not tripping the system. I had a V6 car once only firing 5 cylinders because the spark plug boot was shorting to the head, and the computer never had a clue. Now if it was a newer one like my '07, I'm pretty sure the computer is involved in EVERYTHING and would freak at the slightest trouble.
